The only thing I would add is a list of all the medications your child is taking. You probably won't ever need it, but in case of an emergency, it would be a good thing to have.

A wonderful/nurse friend of ours told my daughter (30 yrs old) to find out what your doctor considers dangerous then measure it every time. It's amazing how little blood there really is when it's measured. This has helped my daughter
