Hypertonic Saline 4% ??

Hi Everybody,

I saw my pulmonologist last week and mentioned to him the idea of trying hypertonic saline. He thought that it was a really good idea. Why in all the years I have been seeing him he never suggested it is beyond me, but I'm happy that he was very open to the idea.

What puzzles me is that he wrote a prescription for 4%-5% hypertonic saline and said that whichever the pharmacy can make is fine. They made 4%. I got the impression that most people are using 7% or 9%, but he said that was too much. I am supposed to nebulize 10cc twice a day.

Do any of you use 4% or something close to it? And do you nebulize 10cc's? It seems like an awful lot to me.

Also, how long do you wait after taking a bronchodilater (albuterol or something of that nature) before using the hypertonic saline?

I believe the lowest dose it comes in is 3% which is what i started doing 3 weeks ago. I use 4 ml (4cc's) at a time. I am no expert but 10 cc's sounds like too much at one time to me.
I do my Albuterol first, then Hypertonic Saline, then Pulmozyme, then Tobi last (if i am on Tobi at that time).

Pre-Tx I used HTS and I have used the 3% and the 10%. The 10% was way too much and I normally just stuck with the 3% and I think that it was like 5-10 cc's everytime, which made my throat hurt. And yes, use your albuterol or whatever first.

I use Hypertonic Saline 7%, followed by the Flutter and it's amazing. The 7% percentage is what was used in the two studies that were published resulting in a 56% reduction in exacerbations. It took me 2 days to get used to the HS -- I had to keep shutting the machine off when it seemed too irritating. But I also saw that this was normal according to the literature that I found. Now, it feels comfortable.

What is so great about HS is that on days that I can't feel any congestion, after using it, I cough up so much stuff, I realize that pre-H.S. days, all that junk just stayed in my chest.

I use Hyper-Sal 7% made by Pari -- which comes in individual 4ML ampules. Maybe your doctor ordered a lower percentage because he thinks the 7% is too irritating for you. The 10cc amount? How long would that take to nebulize? It seems like you'd never get out of the house.

It was great that you brought this up to your doctor. If I were you, I'd ask some more questions. Sometimes I bring my doctors research articles and ask their opinions so we can find things that are right for me.

By the way, I use my bronchodilator, Intal (now only available in spray form), then H.S., followed by the Flutter.
