Is Prandin advised For CFRD?

Hi All, Unforntunately I had to switch health care providers recently and now have an Endocrinist (sp) that has no know knowledge of CFRD. I've noticed most patients use injected insulin to lower their glucose levels and wonder if my doc's choice of Prandin is wise? I'm 42 and just learning about this glucose level roller coaster we experience. Thks for your opinions/advice.--- Paul

Hi Paul,I am 37 and also use Prandin. My endocrinologist Rx Prandin for me. I think alot of CF Dr's may not be thrilled about it, because any information written about CFRD states that injected insulin is ultimately the best, but Prandin seems to work for me for the time being. I have been advised that this won't always work and that insulin will be necessary at a later time, especially if I ever have a transplant. My endocrinoligist said he had one other CF patient using Prandin & it seemed to work for him, so that's why he decided to try it w/ me. It may be a case by case thing too. Where if you have high sugars, but are at a healthy weight, Prandin will do OK to just control sugars, whereas if you are underweight & can't gain weight & also run high sugars, then insulin would work best to offset the sugars you would need to eat to gain some weight. I happen to be at a good weight, but have the high sugars, so Prandin works well for me. One thing that I have found w/ Prandin is; about 2 1/2 hours after I've taken it & eaten, my sugars "bottom out" and I have to eat a snack before I get too low of sugars (shaky, disoriented, sweaty, etc). One good thing is to take it 15 minutes or so before I eat, then I don't "bottom out" as badly. im taking it thats a tablet? if your diabetes isnt that bad tablets are ok, but insulin is the best when it gets out of control. i feel sorry for you not having an endo that doesnt know about cfrd. i had a bunch of idiotic nurses on the ward when i had steroid induced diabetes telling me all this stuff about regular diabetes and how i wont be able to do this that and it made me REALLY upset. in the end my docs and the diabetic educator said to ignore them, they didnt have a clue what they were on about.

Thanks for the speedy replies!!! This makes me feel better 'bout the direction i'm being taken now. So far my weight is good and A1C is 7.8, but like most CFRD'ers, my appetite is all over the place, many times I'm totally bonked after eating, and it seems I can't concentrate or think at times. The fuzzy thinking and sleepiness is the most frustrasting for me, so that's why I'm trying to get some help. The other main concern I had was I did not know if Prandin causes further damage to the Pancreas or it's islets. So once again thanks, the info really helps!---- Paul
