Painful rash/positive rheumatoid factor

I've had this unusual rash for about the last 6 or 7 years. It comes and it goes. It starts on my feet and then slowly crawls up to my legs and on rare occasions, my thighs. It can be very painful and itchy. I first noticed it when I was wearing high heels. So I stopped doing that. I have a photo of my feet after wearing heels and the rash looks like blood has just pooled in my feet. But now I notice it when I wear flats and I don't put my feet up. I also try to keep my feel elevated or stay off my feet for any lengthy periods of time.

My rheumatologist ran some tests years ago and said that I had a positive rheumatoid factor however, I did not have arthritis. Although I do experience joint pain in my knees and fingers when I get sick.

My concern is that I just had my worst break out this weekend. I wore ballet flats for a couple of hours but didn't remember to elevate my feet. Usually this might cause some problems or a rash, but it happened much quicker and much more severely. My feet looked like there was blood pooled in them. My ankles were swollen and I can barely walk. I've spent the last 24 hours with my feet elevated higher than the rest of me. There is some improvement but not a lot. I am still in a lot of pain and when I try to walk I am very shaky on my feet. I recently have had a lung infection that we've been trying to overcome for about 7 weeks. The doctor said that I am as good as I am going to get now. I don't feel too bad breathing wise, but am very concerned about the severity of this inflammation.

I also take Plaqunil and can only take Tylenol because I have kidney problems. So unfortunately, the Ibuprofen or any good pain meds are out of the question. I just finished my first course of Prednisone at 40 mg a day for a week. But the doctor decided to stop it because it wasn't helping. She said I wasn't on it long enough to have to worry about tapering off it.

I would appreciate any advice that anyone has or a message from someone in the same circumstances. I've never felt so bad before that I had trouble walking. I'm a little concerned.
