Sinus Surgery

Okay well.

I went to the ENT today basically to look into the nose bleeds I have been having. Well he changed my nose spray .. yet again and said just to keep using the saline spray until the nose bleeds stop. They have lessened quite a bit so that is a good thing.

Well onto the sinus surgery thing. He said that judging by my last CT Scan of my sinuses that he thinks I need surgery to clean my sinuses out. He said that 4 of my sinuses were "Extensively" blocked and that the person that wrote the report questioned whether or not I had a fungus or mold growing in my sinuses (woohoo for me). He said that basically any person/doctor in the country that saw my CT Scan would recommend I have surgery. He said he wanted me to think on my decision of whether I was interested in having the surgery done or not and explained to me what he would do. He said he would order another CT Scan if and when I decided I wanted to have the surgery - he would want the scan done within a month or 2 of the surgery date that way it is more recent and he knows exactly what he would need to do.

Now my questions. I am wondering how many of you have had sinus surgery (I am thinking quite a few) as I never have and the idea of it makes me nervous.

What was your lung function was when you had surgery? My lung function is currently FVC 40% and FEV 30% and that has me concerned - mainly as to possible complications or difficulties I might have afterwards.

Did you have any problems during or after the procedure when you were recovering?

Were you admitted to the hospital and the surgery performed as inpatient or was it done as outpatient and you go home that day? Were you put on any antibiotics before, during, after the procedure? What about any steroids?

How long was your recovery time? What did it feel like afterwards in your nose? Was it difficult to breathe through your nose afterwards, if so for how long?

I am not going to be having a polyp removed if I decide to do this - at least he did not mention that - I am just going to be having my sinuses cleaned out and the blockages cleared. So if you want to offer experiences with surgery on polyps feel free, but please specify if you had a polyp removed or just sinuses cleaned out. I would greatly appreciate that.

Also when did you have your surgery done? Meaning did you wait until you were just getting off of IV antibiotics - when you knew you were at your best or did you just have it done? I am considering waiting until my next hospitalization in January to have this done (after I have been on IVs a little over a week) so that I know I am at my best.

What was your lung function was when you had surgery?
Lung functions were all over the board. I've had 5 or so, so they've been up and down and around and around.

Did you have any problems during or after the procedure when you were recovering?
I apparently had the gift of gab, particularly cussing afterwards.
I've always been amusing after coming out from under anesthetics, or so I'm told.
At least I didn't flip the nurses the bird after it.
..That was after transplant instead.

Before transplant unless I was already hospitalized it was an in and out thing. They'd give me antibiotics leading up to the surgery.
After transplant they kept me over night and gave me antibiotics before (pill) and afterwards (IV) just to be safe.

How long was your recovery time? What did it feel like afterwards in your nose? Was it difficult to breathe through your nose afterwards, if so for how long?
I'm up the day of, so it's quick. Afterwards, you feel clear. No.
Though warning the first time you have it done it will knock you flat on your a@s.
But even then the drugs are good, so you don't feel anything.

As far as I know that sounds like removing polyps.
If it's not I'd be surprised.

Also when did you have your surgery done?
12, 13? Maybe 14.

Over all:
Positive: It will clear up your sinuses, you will be able to breathe again, it will get rid of sinus headaches, etc. etc. etc.
Negative: It hurts (duh it's surgery), if you get it done once then it will more than likely become a semi regular thing, but that's unavoidable.

Well I am not thinking I have polyp issues because he did not mention polyps. He also said that he does this type of surgery alot of people with chronic severe allergies and chronic sinusitus issues. He said that 4 of my 8 sinuses are full and infected/blocked with secretions due to the nature of CF.

That may mean polyps - but I didn't take it that way when he was talking to me. I will have to ask him when I have my next appointment.

I've had 4 sinus surgeries (I think) I don't remember exactly. They've been very spread out over the years. I've had surgery to remove polyps and other times just to clean 'em out.

The first two surgeries I had when I was 16, then 18 or so, and they were to cut little "windows" in the sides of my sinuses, basically between my eyes. One year they cut one window, a few years later they cut the second window. These windows were made to allow my sinuses to drain easily. I think that's why I constantly have to blow my nose! Would make sense. They were also to reduce the amount of sinus headaches, and it worked. For these my FEV1 would have been in the mid to high 90s. No problems occured.

When I was around 26 yrs old, I had a sinus drainage surgery and removed a polyp. I was probably in the high 80s FEV1. No problems occured, but there was more gunk in there then they anticipated. This helped with the headaches. I was scheduled specifically for this one, no tune ups.

My most recent sinus surgery was for another clean out. This was when I was 31 yrs old and probably in the 60s FEV1 range. I was having headaches again, they took a CT scan and saw I need to have them cleaned out. I don't recal ever having any problems with any of my surgeries. I would imagine the doctors would tell you if you did right?

After ever one I had to use Ocean Spray and a steroid spray. I was never very good about taking them though. I took them at the beginning and then stopped when I felt better.

The only time I remember it actually hurting afterwards was when they cur those little windows. Other wise, for drainage, they're not cutting annything so it shouldn't be too painful. I think they have to call it a surgery b/c they put you under, but they're NOT cutting, so recovery should be quick. They just suck everything up in like a vacum or something. Really gross but effective.

Let me know if I can help anymore than that. I can't think of anything else. I think I'm losing my mind, they sucked out more then the gunk in my sinuses!

The first time I had it (age 13), it did knock me on by booty. Now it is a quick in and out kinda thing. I go to same day surgery or if I am in for IV's beforehand I go down to the OR and come back up. The first few surgeries took up to three hours and the last couple took like 1.5-2 hours. One time my ENT guy gave me a "nose job" and fixed my crazy septum. I guess I am used to the pain now and only take a few Tylenol (if I am in for same day)..if I am admitted then I sometimes ask for something a little stronger because it does feel a little uncomfortable for a few hours after.
My last surgey was almost a year ago. After each surgery I am put on Afrin nasal spray for 3-4 days. He rarely sends me home on oral antibiotics although with the first surgery my CF and ENT dr's decided to tune me up for the surgery because they weren't sure how my lungs would take it. The last few times I would say my FEV's are in the 40's-60's and I come out just fine. A few times I have had to stay on o2 for about an hour to get my SAT's up...that is with not just my sinus surgeries.

I am curious and maybe you can ask your ENT doc if he is going to do the FESS procedure..this is like an endoscopic tool that they can use and watch on the screen while he is up in the sinuses. My guy has to special order that each time..once he wanted to do the surgery and thought he'd do it the old fashion way but then rescheduled because the FESS is much more precise.

I think you will do just fine with the surgery and you will feel MUCH better up there about a week after everything is kinda healed and unswollen. Plus look at all the stuff he can get out of there that is harboring infection in your lungs.

I think you will do just fine with the surgery and you will feel MUCH better up there about a week after everything is kinda healed and unswollen. Plus look at all the stuff he can get out of there that is harboring infection in your lungs.

Amy - to answer your question I think he put me on Nasacort - I haven't picked up the script yet but he thought we would try that again.

Kelly and Em - Thanks a ton for sharing your experiences. The doc told me today that the procedure he would do would involve me being under for about a 1 hr. He said he would probably keep me inpatient for a few days and that he would want to have ome on IV antibiotics for a few days afterwards too. He is wanting to get coordinated with my pulmonologist that way they can keep an eye on me in hospital after the surgery to make sure everything stays okay with my lungs.

I really appreciate hearing the experiences you guys have had. It helps to make me feel better about the prospect of the surgery. I am leaning more towards it right now, but am still weighing everything.
