Which NEBULISER (+jet compressor, ultrasonic etc.) is the BEST

Hi,

The market is now full of many (new) nebulisers, jet compressors and ultrasonic nebulisers too...
Which nebuliser do you find the best? What are your experiences with it?

What for a medication can you inhale in it?
Is the cleaning and desinfection simple?
How quickly and effective is it? 5ml saline solution - 5ml albuterol takes ??minutes, and the delivered dose is about ?? %
How much does it cost?
Your comments, advantages, disadvantages ...
Please post here a link to the device

This should be a little database of compressors and nebulisers which we use and their simple description...I hope it will help us if we will have to buy new nebuliser or compressor...

I have used or use Pari Master + Pari LC PLUS and Pari LL nebuliser (since 1993), Pari Walkboy(since 1995), Nebuflaem F 2000 (since 2004) and Pari Eflow Rapid (since 2005), respectively. Pari Master is 2 years and Pari Walkboy about 4 years in my cupboard...I find Pari Eflow Rapid absolutely the most expensive, most effective and most comfortable, simply the best...but I have problems with the gradual lower output (cloged microscopic laser made holes in aerosol head, by rests of medicament solutions) Maybe I will try some ultrasonic nebuliser, its quit, light and fast...only Pulmozyme is not recomended to nebulise in US nebs...but which ones?

Pari Eflow Rapid (europen version) :
Colymicin, Amiloride, Hypertonic saline, Pulmozyme, Saline solution, TOBI...
Can be boiled 10-15 min., autoclaved (121 C per 20 min), cleaning after therapy under tap water with some detergent, and desinfection by boiling etc.
5ml saline solution takes 8-10 min. 5 ml albuterol takes 8-10 min. Pulmozyme (2.5 ml) takes 3-4 min. Delivered dose is high (there are very small rests of medication in the nebuliser if you want - 0,5 ml...80%?)
In Europe it cost about 700 EURO
Its compact (bag, adapters to all countries etc.), quite, quick, portable, effective, very comfortable, BUT its expensive, you have to change one part of it (aerosol head) every 6 month (60 EURO), because it lost its output, you have to change the nebuliser set once a year...(like Pari LC plus)

Pari Master + Pari LC PLUS and Pari LL
Colymicin, Amiloride, Hypertonic saline, Pulmozyme, Saline solution, TOBI...
Can be boiled 10-15 min., autoclaved (121 C per 20 min), cleaning after therapy under tap water with some detergent, and desinfection by boiling etc.
With Pari LC PLUS nebuliser 5ml saline solution takes 15 min. 5 ml albuterol takes 15 min. Pulmozyme (2.5 ml) takes 10 min. With LL nebuliser (intermittent nebulisation) it is about 3-4 min longer. Delivered dose is lower than with Pari eflow, the rest is about 0.5-1 ml 60%?
In Europe it cost about 400 EURO.
Powerfull compressor, very strong and durable, BUT noisy, big, heavy

PARI WALKBOY compressor with the PARI LC PLUS nebuliser
Colymicin, Amiloride, Hypertonic saline, Pulmozyme, Saline solution, TOBI...
Can be boiled 10-15 min., autoclaved (121 C per 20 min), cleaning after therapy under tap water with some detergent, and desinfection by boiling etc.
With Pari LC PLUS nebuliser 5ml saline solution takes 20-25 min. 5 ml albuterol takes 20-25 min. Pulmozyme (2.5 ml) takes 15-18 min. Delivered dose is lower than with Pari eflow, the rest is about 0.5-1 ml 60%?
In Europe it cost about 320 EURO
Portable, quiter as Pari Master, small, light, battery charging, compact, BUT slowly, only with battery or adapter, only 40-45 minutes on one charging...
Link : Pari Walkboy

Nebuflaem F 2000 compressor with RAPIDFLAEM 4 nebuliser (FlaemNuova)
Colymicin, Amiloride, Hypertonic saline, Pulmozyme, Saline solution, TOBI...
Can be boiled 10-15 min., autoclaved (121 C per 20 min), cleaning after therapy under tap water with some detergent, and desinfection by boiling etc.
5ml saline solution takes 10-12 min. 5 ml albuterol takes 10-12 min. Pulmozyme (2.5 ml) takes 7-8 min. Delivered dose is lower than with Pari eflow, the rest is about 0.5-1 ml 60%?
In Europe it cost about 220 EURO
Cheap, Powerfull compressor, smaller than Pari Master, ligther as Pari Master, faster than Pari Master, BUT also noisy, big and heavy...

Is the eflow available here in the US? It is highly possible that I will be using inhaled Colistin on a routine basis and would like to use the eflow. For those of you that are using it - if you dont mind sharing how much does this unit cost? Also how long does it take for you to do a treatment using Colistin?

It's available in the US - but very expensive. It's approved for Colistin and apparently works very well.

Hi,

the european version "Pari Eflow Rapid" european Eflowis absolutely another device than US "Pari Eflow SCF" version US Eflow version, the europen version is aproved to Pulmozyme and TOBI...US version cost about 3000 Dollars, European about 1000 Dollars...5ml Colistine takes about 7-8 minutes, great is that it not make foam like in Pari LC PLUS and other jet nebulisers...

Just ordered my eflow rapid today, cost me ?700, which in my opinon is well worth it. It will make doing my treatments easier, quieter and quicker. Travelling will be so much easier too!

I have the Pari Eflow Rapid and love it :) We have a special program overhere as of this year where everyone who nebulizes Colistin will get the Eflow. 3 ml Colistin takes only 4 minutes. I will get another set soon so i can do my Pulmozyme and HS with the Eflow too.

How is everyone getting the eflow in the US? I am assuming that a prescription is required. I would like to use it for inhaling Colistin. My doc doesnt have any other patients that are using it and he was suppose to get more info for me but I havent heard back.

There is a Pharmacy in lubbock Texas (of all places) that works with various cf clinics. Eflow is much faster and delivers the meds more effectively at a higher concentration which means that the pharmacy has to mix the tobi and colistin in a smaller dosage.....

So if Tobi is 4cc with a regular neb, it must be 2 cc with eflow......of course, this pharmacy does EVERTHING for you...and they mail everything overnight.

The place is called IV Solutions in Lubbock, Texas. The Eflow has been the coolest thing I have ever had for breathing treatments.

Tobi takes me 3 minutes....albuterol takes 2 minutes.

mandy
call them and they will let you know what you need to do to get the eflow.

PS the reason the eflow is not approved by the cf foundation is because the makers of TOBI filed suit against the makers of eflow because they were afraid the device would make their medicine obsolete....so instead of having the patients quality of care in mind, they were pissed and decided to have an injuction to keep eflow off the market. The makers of TOBI did not want to have to change the amount of the drug used because that is less money for them!!! Eflow only requires half the dosage because the delivery system is so much better.

Hi Amy-
Actually, I signed my name in that last post..it was mandy.
Anyway, just google the "eflow lawsuit " with Chiron.....I was working with Source CF and can tell you WITHOUT A DOUBT Chiron did come after us and it was because they did not want anyone fooling with their product......

They said because the eflow was such a better delivery system too much TOBI could be delivered and cause hearing and kidney damage.....that was their "concern" If you talk to the lawyers involved you will learn that it was a legal move based solely on monetary concern..... to put Source CF out of business.

It is not a theory...that is what happened.

Well I'll tell you that I have substantial hearing loss from TOBI, so Chiron's concern could be legit.

I will look into your claims.

HI Amy,
I didn't mean to be rude last night, but I was hurt be your tone. My "theory" is not conjecture....I didn't make up what happened. Chiron is a pharmaceutical company and their bottom line is money....yes, they help people, but they are a product of capitalism like any other company.

As for the issue with having to lower the dosage for eflow, well, Chiron did not want other companies (not just Source Cf) messing with their product. The makers of eflow were well aware that the dosage had to be lowered which meant that compounding pharmacies would have to mix tobramyacin at a lower dosage. Chiron, for obvious reasons, did not want that to happen.

Also, the more people that heard about eflow and wanted to use it.....cfers who heard their antibiotics inhale could take 3 minutes vs. 15 to 20, wnated in on the action. Every persn using eflow was one less person using TOBI.....and TOBI cannot be used in an eflow.

I respect you and I value your knowledge of antibiotics and their specific usage....everytime I question something I look up to see what you have said or are saying. I just happen to know A LOT about this particular topic and the way you addressed me was hurtful.

Hey Amy-
sorry, i think I might be pmsing....I, too, tend to do research to before I jump on a band-wagon. I just am VERY familiar with the way huge companies work....the bottom line, the bottom line, the bottom line...can't help others if they can't help themselves.

hope you are well.
