I was prescribed this as a replacement for Tobi, as both the podhaler and nebulised versions have worse side effects than perceived benefit.

First it was in the left shoulder for over a year. Then it mysteriously got better. About 10 months ago it started again in the right shoulder.
No reason. No solution.

And your annual glucose tolerance test, by this do you mean the type where one has blood drawn post-fast, consumes a large quantity of dextrose, waits two hours, then has blood drawn again?
What were your pre and post glucose numbers?
If your fasting glucose (first test) is anything above 7...
The most obvious which hasn't been mentioned yet: CFrD.
Did your headache come on due to the consumption of carbohydrates?
Are you tired mostly after eating?
Are you always hungry?
I would encourage you to pursue the fact that you may have developed CFrD.
I'm Canadian and when I travel to the US, I always get a letter from my doctor / CF Clinic, on letterhead, with a list of prescriptions and diagnosis. Not that I offer it up, but rather that I have it, just in case. And that's for either direction.

Coughing and blood: this may not necessarily mean infection. And the source of bleeding can be anywhere in the airway-lung. A "mild" bleeding is merely caused by irritation and dryness and a broken vessel in the trachea or upper...

You will get a six-pack abs (or other muscle definition) just like anyone else: proper diet and EXERCISE.
I have CF, I'm 36. I too struggled with this for two reasons:
With CF, exercise is discouraged, high fat diet is encouraged.
