I tried to reply to your PM, but I'm not sure it worked. You cannot pass both of your copies to your child. One comes from dad and one from mom, always.
Could you have undiagnosed CF, possibly. If you would feel better being tested, then go for it, but you can't have given your daughter CF...

No, I must have signed paperwork in my hand to honor an OOHDNR (out of hospital do not resuscitate). I have had cases where I was forced to do CPR/ACLS because they did not have the DNR form on hand.
Most people keep them on the fridge with a magnet, we usually look there if the patient is...

For muscle strain, Skelaxin is another good one that doesn't knock you out like Flexeril.
An x-ray needs shot in his case to rule out a host of other things.

Actually a good song for CF would be Genetic Emancipation from Repo! the Genetic Opera.
Part of the lyrics: You, I've mistaken for destiny, but the truth is my legacy is not up to my genes. True, though the imprint is deep in me, it will always me up to me.
Look it up on youtube.
I think something non-traditional might be very difficult to spot. I, for one, didn't even know they were available, as I've never seen one. So, I wouldn't know to check and that's probably true for most medics or first responders. This would be especially true in a traumatic injury situation...

I had a similar problem in school and my doc wrote a very stern letter. While not overtly threatening it, in effect, said that if the school did not follow his orders he would pursue legal action. I had no problems after that.
You are responsible for following your doctor's orders and anyone...

It is nice to have, but usually on the back end of a call. Meaning, I can photocopy your list rather than ask you all that information, or I'll have it for my report if you are unresponsive. Many people, especially the elderly, have print outs of their history, meds and allergies that their kids...

Jen, do you know what format it outputs as (I'm assuming .pdf)?
I don't mean to be a downer here, but having worked pre-hospital and in the ED I can tell you that it's nice to know a full medical and surgical history along with medications and allergies. However, I'm not sure somebody will...
