Like I said in the pregnancy section, I do remember reading about the boy who did get high tobra levels in his system while being on TOBI. This was on Cystic-L many years ago. His mom was complaining of his symptoms and someone suggested getting the doctor to do a blood level and sure enough he did have it in his system in very high levels. I have not studied this at all but I'd have to imagine that some must get in the bloodstream at times. This is from the tobi website (tobitime.com) in the prescribing info pdf:
"Serum Concentrations: The average serum concentration of tobramycin one hour after inhalation of a single 300-mg dose of TOBI® by cystic fibrosis patients was 0.95 ?g/mL. After 20 weeks of therapy on the TOBI® regimen, the average serum tobramycin concentration one hour after dosing was 1.05 ?g/mL."
That is a very small amount but it is still significant in showing that it DOES get into our blood stream and doesn't just stay in the lungs (which seems to be the common belief, even amongst our doctors). So if someone is having ototoxic symptoms while on TOBI don't dismiss it however rare it might be to have that complication.
