Bad enough for a tune-up?

Hey everyone,

I had a clinic appointment on the 8th (almost two weeks ago). Weight down 2kgs (about 4.5pounds) and PFTs down from when i was discharged out of hospital in December. Down to 96% from 107%. I usually go back every 3 months but the doc said to come back in 6 weeks, and if my weight and lung function isn't better I should be admitted. As I said I was only in hospital 2 months ago for a tune-up, and my lung function isn't bad considering. I don't feel really bad or anything, obviously not as good as when I got out of hospital but that's normal and all. I guess I was just wondering what everyone thought, like I know its down a little but is it worth an admission do you think?

Advice would be much appreciated <img src="i/expressions/face-icon-small-smile.gif" border="0">

At a CFF meeting I recently attended, one of the things that was stressed as a vital component to keeping us healthier longer, was to favor hospital admission for a "tuneup" over a course of oral meds done at home. Although your FEV1 is still remarkably high, the fact that you've lost weight sends up a red flag that you may be fighting an infection.

I've learned (sometimes the hard way) that a slight infection can get complicated in a very short period of time. A lot can happen in 6 weeks' time. I'm surprised your doctor would even have you wait 6 weeks before reassessing the need for a hospital admission. But then, I don't know your situation or what your daily meds involve (for example, maybe you're due to start TOBI in a couple weeks or something).

Personally, I have tried everything in my power to resist admission at times, and it has ultimately come back to bite me. I would give myself another week, two at the most, and then schedule a visit with the doc to talk about admission.

Best wishes as you decide which road is the right one for you right now!

I can see both sides to this easily because while yah you want to keep your lungs healthy you also can't really spend your life on IV's...and then theres the resistance issue..and do you actually go IN the hospital because germ wise I think that's rather dangerous.

And also I suppose there's the other factor of do you know something your Dr. doesn't? Have been a real busy lately and maybe missing some treatments or skimping on snacks or anything. It happens to all of us, that is just a fact of life but if your doc doesn't know that he's only going to think the only possible explanation is infection...when maybe it's just...you've been preoccupied?

And then for me I also know that I never ever stayed at the level I was when they discharged me. I knew as soon as I got off the iv drugs and they were out of my system with in a week or two I would drop a bit. Allergies and my usual general irritation would set up shop again causing a dip in PFT's. For me that's just the way it was.

Maybe you should talk to your Dr. a bit more...sort of explain that you know you are feeling good and maybe talk about your life style and what you want your approach to be here. Are you going to do iv's every 3 months to prevent any drop in function or are you going to do iv's on more of an infection based mode? Or maybe you want to compromise and do scheduled tune-ups evry 6 months...I don't know but I think you should maybe try opening up the discussion of how you want your general approach to be?

I too have been bitten because I have tried the oral antibiotic route, did that from Thanksgiving until almost Christmas and just got worse and worse, felt downright toxic. By the time I was admitted, I had lost 10 lbs...so now, I just bite the bullet and get admitted when the doc says to. With our lungs, a simple infection can get worse so much quicker than a normal person's. Hopefully you put the weight back on and the next check up is better and an admission is avoidable in your case. Definitely watch your body, if you find that you are still losing weight and continue to not feel yourself, see your doc sooner than the 6 weeks.
