CF accredited Care Centers Addresses

I'm trying to track down a document with all the MAILING addresses for all the accredited CF centers. I've contacted the CFF but they tell me their copy is only for CFF and that they can not distribute it <img src="i/expressions/face-icon-small-sad.gif" border="0">.

I've complied a word document of the names of all the care centers (taken from the cff.org site) but they do NOT have complete mailing addresses, just name, city, state and zip.

Does anyone have access to this information? If you do, could you please get in touch with me???? services@dafcf.org

i want to say that i have a reference book thing with the addresses in the back of it of all of them, the book is a few years old though. I'm not sure if it just has the city/state or if it has the addresses i'll have to look when i get home. Maybe someone else has the same book? its one i got from the scandishake people as a freebe thing, i want to say its black and orange, a paperback thing about the size of a small phonebook

I have the same book as Robert does... it was given us when Emily was dx'ed. 'An Intro to CF for Patients and Families' by James Cummingham and Lynn Taussig. It has all of the contact info in the back appendix for clinics by state (Address, phone numbers, clinic director) and it was up to date at least up to whenever the book was written... we have the 5th edition, copyright 2003.

eta: our edition is a white and blue book; also sponsored by Scandipharm (and the CFF.)
