I almost took a job in Riyadh and my docs stateside assured total support in consulting with local docs. Maybe you can work out an arrangement like that?
I lived in Italy for close to a year and took all mymeds and everything with me. Though Italy is a far cry from rural Africa my local docs had contact with a reputable cf doc a couple hours from where i was living and so I had a connection there if i needed anything (i did end up needing some abx while i was there). LIke Jonathan said, i think you should see what kind of connections your docs in the states have over there somewhere close.
what do you do for airway clearance? I would imagine in rural africa that electricity is not reliable so the vest would be difficult. do you have an acapella or flutter?
