In my state you can get them where you get your license tags/plates for your automobile.
Of course you need something in writing from your doctor stating why you are disabled first.
You should be able to go to your county tag renewal website & download an application for disabled tags/stickers and take that to your doctor, then when he's completed his part, you then take it to the tag office.
At least that's how it works here.
As for getting a disabled parking spot at the local WalMart, I say forget it....they're always full when I want to park there.

I got one from my state represenatative. I called the driver's bureau and they sent me a form for my doctor to fill out. I had to time it well though because they can be hard to get. I got it when my lung function was 30%. Well it is now 50% but they didn't ask when i renewed it so that was good. I have a hanging thing and make sure you hide it when you aren't using it. People will break into cars and take them!

I got mine by filling out the application for the DMV and giving them my doctor's note. Here's an odd piece of trivia from the CA DMV--if you are applying in person for a disabled placard and you DON'T HAVE HANDS, you don't need a doctor's note stating your disability. HA! (I bet they still make you stand in line though...)

Hehehe. So true. And why are there so many disabled parking spots at the skating rink? I know, I know...don't assume. But still, it sort of makes me snicker a little. That and the fact that the skating rink I'm used to is next door to a blood bank. Ha!
