A crossover, Randomized, Crontrolled Trial of Dornase Alfa before versus after physiotherapy in CF

Fitzgerald, et al. PEDIATRICS Vol. 116 No. 4 October 2005, pp. e549-e554

Dornase alfa is equally efficacious when delivered before or after physiotherapy/PEP in patients with CF.

Patients who are colonized with PA may derive more improvement in FEV1 when dornase alfa is delivered after physiotherapy/PEP

Amy, that article is pretty interesting. My dd does not have PA, so I guess it really doesnt matter if she does her Pulmozyme before or after. I do have a question....when the dr put her on Pulmozyme while in the hospital, he said she could do it with her vest treatment but after her albuterol....my question is, should we be waiting for 30 minutes like the article said?

Past studies have instructed us to:
1. Dialator (albuterol)
2. Pulmozyme
3. Hypertonic Saline (h/s)
4. Vest
5. Inhaled Antibiotic (a/b)

However, at my last clinic appt I was talking with our pharacologist, since I added h/s to the list.

He said recent studies have shown that it is ok to do pulmozyme and then H/S during the vest, that it can be more efficient in getting the junk out.
Continue using dialator first and a/b last though

For me, however, it varies. Sometimes I can't take deep breaths with the vest, so I think it's a little fruitless trying to inhale meds at the same time. Par for course, it all varies person to person.

Like Sally, this is the order I take my meds and I do my HS during the vest because I find the combination extremely effective for me with bringing up the junk as opposed to doing them seperately.
