My SW told me it can be downloaded from their site. (I've also included the link below)

Apparently it's hit-and-miss with who's received theirs and who hasn't. Ghpp is letting the center's know who hasn't turned in their application, and, go figure, some on the list weren't even mailed one!

So, if your's is about due and you haven't heard from them, don't wait!

Because of cut-backs, ghpp has dropped the ball on getting some applications out, in some cases. And,
<b>if yours is past due, they WILL deny coverage. </b>

<b>It's important to be pro-active - if yours is coming due and you haven't heard from them, don't wait - download the application and turn it in (also, it's always a good idea to make copies and send registered.)</b>

Utilizing the GHPP web site is the best vehicle to get ones application, the employees there are either so busy or so understaffed they are neither returning calls or following up on messages.
