Inhaled Aztreanam

For those of you who are participating in the EAP for Aztreanam Lysine, what do you cycle with it? I know it is 4 weeks on then 4 weeks off. Do you do inhaled Tobi during those 4 weeks off? or do you just do an antibiotic by mouth and stop Tobi altogether? Also, we were thinking of doing a tune-up in January. Would it be best to do a tune up then start the Aztreanam or should I forget about the tune-up. I know you are all thinking that these are questions I should be asking my doc, well guess what, I have asked some of those questions and I am having somewhat of a communication breakdown. I feel kind of alone in making these decisions and I just want some input. I know a lot depends on my cultures and yadayadayad; but, having some input from others so when I do make decisions with the doc I am better informed always make me feel better.

Speaking from my personal experience, I would probably NOT do a cleanout AND aztreonam. What happened to me was my PFT's were down two weeks before I was due to start the Aztreonam, so my doc wanted to see me in a month (so I would have had two weeks of AZ when I was to redo PFT's). Well my PFT's were still down then, so the last two weeks of my AZ cycle, I was also on Ceftax, Tobra and Vanco by IV. I found that was a lot of my body and ended up only doing AZ twice a day instead of 3. My reasoning was first of all that it was a lot of abx all at once, and secondly, I was so wiped out from doing the IV's that a lot of times, I would forget the AZ.

I just got my PICC pulled today, and usually was NOT cycling annything else with the AZ (b/c I was in earlier studies)...BUT I found that I was much healthier in the on months, and usually just by chance ended up on oral Zyvox and/or two weeks of TOBI. Right now though, my doctor gave me Zyvox to get me back on track after the IV's.

If you have any questions, please ask me, I have been on AZ for two years now, so I practically know more than the docs..lol. Some ppl in my clinic hadnt even seen the eflow!

I'm thinkning of starting the AZ after I get off IV's. I won't be doing anything in the off month (probablyl) since I don't tolerate inhaled meds very well.

So how did the AZ work for you Caitlin?

My Dr. wants to me to cycle with the Tobi on my AZ off time. I am also on 2 oral antibiotics along with the inhaled AZ. Since i am always on orals at all times for maintainance, we decided to stick with that along with the inhaled AZ. I am also only doing the inhaled AZ twice a day instead of 3 because it is soooooooooo irritating to my throat , i just can not do it three times daily. My nurse said as long as i do it twice a day, studies have shown it to still be beneficial.
