I usually wait until after I eat to use my vest. I don't vomit often. Most of the time I have a stomach of steel unless I have an extremely bad case of heartburn. I feel for you. Vomitting isn't that bad IMO, for me it's actually a welcome relief from the horrible pre-vomit feelings. Yuck :S !

I think two things can really contribute to this...
1) a loose gastric sfincter muscle caused by GERD, pregnancy or coughing (causing permanent damage). I think sometimes people have to get this fixed before transplant since it can cause aspiration of gastric juices into the new lungs.

2) sort of the same as 1 but just to reinterate. coughing. and I mean the sort of coughing only associated with moderate to severe pulmonary involvement.

I'd be curious to hear what the FEV1 is of most of the people that are talking about experiencing this. My guess is most are 60% FEV1 or lower or had a baby.

Perhaps it's something to talk with your cf doc and your GI about. If you have to do a surgery it would be better to get it done while you are stronger than during your work up for transplant.

The only time I have issues with eating while (or before) vesting is when I have a coughing attack while inhaling hypertonic saline. Just vesting-- no problem.

Sometimes though when my stomach is being super sensative I have to really fight the gag reflex (even if I havent ate for hours) or I do just puke. Obviously any time I am feeling nauseous, its hard to cough (the vest still doesnt really bother me)... coughing really triggers that gag reflex for me.

Interesting Lou Lou, I didnt start vomiting after really bad coughing fits until I got to lower lung function... I mean, I guess it kinda made sense in my head, and was always asked about it at my ped doctors appts.

For me, I just try to do my meds on a completely empty stomach, but yeah, sometimes it's hard to have an empty stomach, especially if you're like me and like to eat all the time.
