MDI vs. Nebulizer

Well our treatment schedule goes as follows;

We begin with the albuterol through the neb for approx. 12 minutes. We then do the hypertonic saline through the neb and wait two minutes then turn on his vest. He then begins to fight us as he does not cough AT ALL with the hyper Sal but he just does not like to breathe it in (and I don't blame him!)

So when I mentioned this to our dr. he thought it was just too long of a treatment for a 10month old guy nad told us to use the MDI for the albuterol. So we have been doing this only for the past few days...it's hard to know if it works the same better or worse as he doesn't seem to be any different! I think I will go back to the neb for everytihgn and only use the MDI when we are running short on time. Our Dr. told us not to use the vest with the Albuterol as it is not as effective? Not really sure though!

1). I'm shocked your albuterol is taking 12 minutes.... how old is your compressor? Are you mixing albuterol with anything else? What is the dose of the albuterol? How many mL's?

Compressors can give out quickly, so it may be time for a new compressor even if it's not 3 years old yet.

If it's PARI, they'll replace it for you for free <img src="i/expressions/face-icon-small-smile.gif" border="0">

2). The man who created the Vest, Dr. Warwick, absoultely recommends doing albuterol and/or HTS while vesting. So smack your doc over the head for not keeping up with the latest thinking.

Oh, and our albuteral (3ml) takes about 8min with our Pari (Proneb II) compressor, by way of comparison.

Ditto about checking out a different compressor. DS uses a vial of atrovent mixed with .5 ml of albuterol and it takes about 12-15 minutes maximum.

We had a faulty pari compressor when he was a baby -- brand new,but the albuterol/atrovent took about 20 minutes and then his tobi took 45-50 minutes. Way too long, so he wasn't deriving as much benefit from the meds.
