It varies by state. So you need to look at your State's website.
In Mass. it would cover all of the things you ask about.

From Texas health and human services:

What's covered?
In most cases, Medicaid pays for doctors' services, laboratory and X-ray charges, medicines, nursing facility and hospital services, family planning, eyeglasses, hearing aids, ambulance, podiatry, chiropractic, maternity, nurse midwife, and other health care services.

People who are elderly or have a disability who need help with daily activities may be able to receive services in their homes. The state also offers some non-Medicaid community care services.

Medicaid often pays for regular checkups, especially for children. In some cases, children may even get to see a dentist. Your Medicaid ID card will tell you what services you can receive, and when regular check-ups are scheduled

Some advice--be FIRM in how you deal with these folks. You're going to learn VERY QUICKLY that things don't happen smoothly with 1, 2 or sometimes even 6 phonecalls. Get a referral to a specialist ASAP and make sure the referral covers more than just the typical "well child (aka regular checkup)" appointments. It's hard work, but it's a necessary evil. FIGHT LONG, FIGHT HARD, FIGHT WELL.

I work for Arizona's Medicaid program, and all the things you mentioned would be covered. You should also check to see if your state has anything like a special children's program. In AZ for example, there is a program called Children's Rehabilitative Services, and it is in addition to Medicaid. All children in the state with certain diagnoses (and cystic fibrosis is one of them) are eligible to receive services through their special "clinics" in which case all related services are provided during one visit. PM me if you have any problems, and I may be able to help you navigate your state's system. Be persistent. Medicaid children are entitled to a lot of services.
