Neb order during treatments...

It's been a few years since I've really buckled down at home and done my vest and nebs twice a day. I am so proud to say that I haven't missed a treatment in almost six weeks! I have just added HTS to my routine and was curious about the nebulizer order I should use. The RTs at the hospital had no idea and I could call my doc, but I know that someone here will be able to help me.

I do albuterol to start, then Tobi, then Pulmozyme as this is the order my docs at my old, old (haha, two hospitals ago I mean) hospital (like, seriously, five years ago) told me to go by. Is this still true? Also, where should I put the hypertonic saline in? Back in the day they said you shouldn't do too much coughing after you inhale the Pulmozyme... is this still true? I know that studies change everything practically every other week. I am doing 3% and the vial is huge, about how much do you guys do? I am not going to sit with my 10 year old nebulizer for an hour just doing HTS!

Hmmm... Okay, here's what I have been doing:

My doctor told me Xopenex, pulmozyme, wait 15 mins., HS. I did that, but then thought that that doesn't leave the Pulmozyme much time to work (especially for how much it costs- lol j/k . But, seriously, not much time to cut up the old stuff, and it would be working on stuff I was about to cough up anyways...

SO...I switched, and now I do Xopenex, HS, vest, then pulmozyme (and of course, inhaled antibiotic last if I'm on one <img src="i/expressions/face-icon-small-wink.gif" border="0"> ). Seems to be working pretty well! I am pretty clear all day like that. Today, of course, I got in a hurry and did my nebs with my vest, forgot to do my Pulmozyme, and spent the day thinking "Why am I so tired and congested? Am I catching a cold?" It just occured to me that I shortcut my routine today, and now I am paying for it-duh... <img src="i/expressions/face-icon-small-wink.gif" border="0"> .

I have been switching up between the 3% and the 7%.

I get the 3% in the big vials too- I do 4 ml, and put the vial in a zippy in the 'fridge for the next treatment. I don't like to keep a vial more than 12 hrs, though (personal paranoia <img src="i/expressions/face-icon-small-wink.gif" border="0"> ).

You might also want to time your treatments (for each med). 10 years is kinda old for a neb compressor- Mine wore out after about 2 years, and was doing very slow treatments. At least having a back up on hand would be good. I would have been stuck when mine died if I didn't have my Pari Trek (recent purchase, too!) <img src="i/expressions/face-icon-small-wink.gif" border="0">

3ml of my Xopenex takes about 7-8 mins with a Pari LC + and my Proneb Ultra II, if that helps.
