Question about when to take meds...

I just started taking pulmozyme, doing the vest, and am now starting Advair. Right now I do my pulmozyme when I am doing my vest, and that works out well. My question is-can I take the Advair right before/after the pulmozyme? Is it ok to have both of them in my lungs at the same time? Or is it best to do the Advair right when I wake up and right before I go to bed, so I am not doing them too close to the pulmozyme? Or does it even matter? Just curious what others are doing. I am also starting the Hypertonic Saline in June so I'll have to figure out when to do that too!

As for the Advair and the Pulmozyme I keep them a couple hours apart starting with the pulmozyme fiist thing in the morning. Now As for yor Hyp. Saline you'll have to figuer that out on yor oun. it makes me caugh like crazy. Good Luck.

This is what I got from the Cf Foundation website - this is the order in which to do things:

1. airway dilators (albuterol, ventolin, etc)
2. DNase (Pulmozyme)
3. airway clearance techniques (vest, percussor, acapella, etc)
4. antibiotics (Colistin, TOBI, etc)
5. inhaled steriods (advair, symbicort, pulmicort, etc)

You don't have to wait a couple of hours to take the advair after the pulmozyme. As for the hypertonic saline, I'm not too sure where you would put that in. I would imagine after or before the pulmozyme, but I'm not sure. Maybe you should talk to your doc about that, or maybe someone on here would know.

the thing about advair is it takes about a half an hour to even begin working. If you want the "maximum" effect I would take the advair, let it "open" you up and take your neb after an hour or so. The theory being the more your lungs are open the deeper you can get the medicine down.

As for your concerns of meds in the lungs, nope it is fine.

I do my Advair about 1/2 hour after i get up in the morning. Then do my Albuterol ( inhaler) about an hour later, followed by the vest for however long i do it, then right after the vest i do the Pulmozyme. I asked my pharmacist which would be best to do first and he said the Advair should be first, so thats what i do.

Liz' post is a great list.
I haven't started the Hypertonic Saline yet, but was told by my clinic nurse to do it 15 minutes after the albuterol and before the TOBI and treat it as an airway clearance tool. On the list Liz provided, that would put it at step #3.?

I suppose the HS would be before the airway clearance? Do the HS and then do your physio? I just wonder if the saline would be something you wouldn't want to cough up, or is it? If you did it before physio then it would help you clear stuff out, but if you did it after physio then it would sit there maybe.

We don't have Pulmozyme or Dnase here but have been using 5% saline for a number of years. Our doctor reccomend that you use the boncholdialators before using the HS because this help prevent irritation. It's also good to take these before your Physio.
