I have the "The Vest" machine and it's by no means small. But I've flown many many times with it, I just get there a little ahead of time and say "Look, this is about a 15-20 grand piece of medical equipment, I can't check it, I don't need it RIGHT NEXT to me, but it needs to be a carry on". I've never had a problem, it's slowed me down getting through security and been a little embarrassing carrying it on to completely full flights, but it's medical equipment, that's just the way it works. Having a note from the doctor I imagine would greatly help, one of the airlines said they needed a note, but I kindly told them I didn't have one and they needed to deal with it.

The main thing is I've flown dozens and dozens of times in my life and I always hated the "Getting to the plane" part, NOW I not only have my two carry ons, but I ALSO have the vest and my nebulizer machine, so I'm lugging around four things through the entire airport.

We've got the 104 model of the vest and just got the new bag with wheels. We rarely travel and with the first bag, the inner pockets sorta just shredded at the seams. I just emailed them and they popped one in the mail for us.

We flew from Detroit to Tucson in March. I have the "old' 103 model vest and it actually fit in the overhead compartment. I bought a folding cart to lug it through the airports. Going through security was no problem at either airport. I thought everything would be a hassle, but had no problems.
Good luck
