Vesting routine

My boys are fortunate to have a therapist who comes to our house to do CPT everyday. They also have the VEST. They both say that the manual PT is more effective, but I am wondering if they just don't use the vest properly.

What is your routine? How many minutes, settings, coughing?

We do two 30min sessions a day when healthy; 6, 8, 9, 18, 19hz for 6min each. She starts with a pressure of 8 and it goes down to a 4 by the time she's done. She pauses it to cough as needed- pretty frequently- and is pretty good about doing the huffs with the coughing and brings up a lot of junk when she is sick.

We used to use a different set of settings but I switched to the above after reading information about Dr. Warwick's studies on Amy's blog... and these settings do seem more effective for her. (Originally she was prescribed two 20min sessions daily: 10-14hz, 4min each, all at pressure of 4.)

If it's been a long time since they've gotten the vest~ are the jackets the right size, and fitted appropriately?

I use my vest twice daily for 30 minutes sessions. I start at 13/3 and work my way up to 16/6. I end at 14/4. I stop the machine to cough every five minutes.

We just got bigger size vests for them, so they should fit properly.
My kids were prescribed the same thing. I'll try this method.

We just got bigger size vests for them, so they should fit properly.
My kids were prescribed the same thing. I'll try this method.

respirtech quick launch @ 80%.

One note though the reason my old CF doc didnt like the vests is because it was 'passive' and he said that since people were just sitting there they didnt tend to put in the effort required to get something out. You shouldnt just sit there and continue to push the buttons and get through their cycle, if you dont stop to huff and cough all the vest is doing is breaking up the mucuos and allowing it to sit in there broken up.

I am not familiar with the new hill roms but the respirtech pauses every 5 minutes so at a minimum I huff/cough then, I often stop if I feel stuff that is 'ready' to get out.

Also speaking of Amy's blog and Warwicks letter the pressurized cough that I read on there and implemented a long time ago is very effective for me. If I huff until clear, then do the pressure cough, then huff until clear it is the most effective combo I have found during PT.

I think you are wise to get your boys used to alternate options besides just PT because if there is a time, say in college, where they are unable or unwilling to get PT they wont feel like there is no use in doing anything.

Nicole~ I agree that the vest will be of limited value if the person isn't coughing up whatever is being loosened up. Emily coughs up more than she did with cpt. Fortunately, she seems pretty sensitive to the feeling of stuff down there and she wants to cough it out... at clinic visits her dr always wants to see her show them how she huffs and coughs, too to make sure she's doing it right, which I think is a great idea. They need to have a good technique and be aware of how it feels to be clearing mucus from an early age.

The newest HillRom doesn't pause by itself, but it comes with a cord with a squeeze bulb on the end as long as the hoses, that lets you conveniently pause & resume session as needed, without messing up your programmed sessions. You can also stop and start using the on/off buttons w/o messing up your session.

I do flutter valve WITH the vest, so I'm always huffing and coughing non-stop.
I wouldn't suggest this, because it's very rough on the body, but I spent a long time working my way up to it (my brother [also with CF] can't do both at the same time, it's too hard on his lungs).
My program goes all the way from 12 up to 20 (the highest setting) in a matter of about 15 minutes.
