Want to be retested...Genotyped

I was genotyped 11 years ago, and would like to be retested... I would also like to have my 17 & 14 y/o sons typed... I was told by ins. co. to have our drs.write letters to the ins. co. to see if it will be covered.. I would like to know exactly what the name of the test is, so that, that's what is put on the letter.. Thanks for any help... Joni 55 y/o f w c/f
