I'm wondering if the increased coughing is causing pressure on his ear, thus causing the ear pain. He may have fluid building up behind his ear drum as well. I'd have them look at it, because ear pain is horrible, and you don't want a busted ear-drum. They can give you some abx ear drops with an anesthetic in them to numb the inner ear. Let us know what you find out.

Stacey, that is exactly what our cf clinic told me a little while ago. They said to go into our ped and have him take a look because it may be fluid build up. So, we are going in at 3:00. I'll write back with an update. Thanks!
