Who decides Port or PICC?

well i must say my 2 year old has a port and has had it for almost a year and it makes such a wonderfull diffrence. its great. know when she goes to the hospital its only for 3 days and we come home and i do the rest. i love it and so daoes she. she was still a candiate for picc line but a port makes life so much easier.
