inhaled antibiotics

i know with IVs it is very important to keep on a schedule so that a consistent amount of the medicine can be in you at all times. my question is, is this equally important with inhaled antibiotics? i have been doing TOBI twice a day but the times have been really inconsistant.

I was told when I first tried TOBI several years ago to try and do the doses as close to 12 hrs apart as possible. I try, but sometimes go longer and sometimes go shorter usually it ranges between 10-13 hours apart for me. Doing them is key, trying to do them on a schedule I think is important but times can be somewhat flexible (within reason).

We are fairly consistent with DS and his tobi. We get up at 6 a.m. and do his CPT and then he dozes in bed with DH does his tobi, while I shower. Then we do his CPT right away after work/preschool at 5:15-5:30, then tobi before supper. Sometimes if we're busy or are travelling, we end up doing the tobi closer to 9:30 or 10 p.m. at night. Only time we've ever missed a dose is when we've missed starting on time because the CF Services pharmacy claims they've sent his tobi to us and later we find out the customer service LIED, it was never sent and nobody bothered contacting us.

Tobi is such a pain in terms of how it seems to take up all our free time in the evening. And even though it's 28 days on, 28 days off -- I sometimes feel as if we're ALWAYS doing tobi. It's especially hard in the evenings because we're rushing around getting the treatment done, then dinner and by then it's pratically time for bed. Little time to play then bathtime....

I do it at 10pm and 7:30am - too close but like Lindsey said - at least I'm doing them. Maybe when I'm a stay at home mom I'll do them 12 hours apart. or maybe I'll even try to do it at 7:30. you guys inspire me. the thing is I like to finish off my night with TOBI and if I finished lung care at 7:30 I feel like there'd be junk in them before I'd even go to bed. Maybe I'll give it a try. whine whine 7:30 is in the middle of my evening...

I know what you mean about finishing off your night with treatments and if you do them early you feel like there is junk in your lungs again by the time you go to bed. Usually when I do my treatments more than 3 hours before I go to bed I throw an extra Albuterol treatment in right before I lay down. I find it usually helps me breathe a little easier.

When I do TOBI, there's usually a 10-13 hour stretch in between. I try to do it at the exact time every day (12 hours in between), but sometimes it is hard with my schedule. At the very least I am doing them 2x a day and within the 12 hour range. I don't think an hour too soon or too late is a big issue.

you are asking for resistance.

and with CF, that can have huge consequences.

there are very few antibiotics that can be used for PA in the 1st place. And even fewer that can be used out of the hospital.

If you don't use your TOBI on a regular schedule, you will be inducing resistance.

Either don't do TOBI at all, or adhere to it religiously.

Hmm...I'm not sure that all of us adhere to doing TOBI religiously on time, the same time every single day due to schedules or what not. Sure in a perfect world we would be, but I know that's not the case. So I don't think its a case of either do it on time, same time everyday or don't do it at all.

Hi...although 12 hours between doses is most ideal, there is a bit of a window...at least that is what I was taught. Makes sense. I will use an example out of hospital policy. I used to be a nurse, in our proceedure manual, it was okay to have a plus or minus window of 30 minutes to give IV meds. Why? When you have 8 patients all on different schedules, some the same it is nearly impossible for you to be in two places at the same time to give these meds. Pharmacist was okay with that and so was I as long as I got those meds to those patients within that window.
In the home setting, things may come up for you to be an hour late for your TOBI, I admit...it has happened to me, but at least I take it. I would say don't bother taking it if your are going to be grossly non compliant with your TOBI, such as not taking it one day and then deciding to take it a few days and then not....then I say don't bother.
