Jenn....didnt you say that your steroids are cut back? That alone can make a big difference. I thought one of the reasons that your lungs stayed pretty good was because of the steroids for all the other stuff. Your body has been thru a lot & the fact that you are tired from being back to work has much to do with it. No real answers for you since it could be a few things at the moment..

Can you do physio right now?

I believe what made my pfts go up so high last year was my more serious approach to physio, after I learned the correct technique. And, doing hypertonic. Its harsh at first, but you don't have to start with 7%-start lower and work up. See what your body can tolerate.
Like they say-its no big secret to getting your pfts up-physio physio physio. But, for me I honestly did not know the right way to do it. I was doing the vest wrong for years. When I learned how to do it right, I really stuck with it. Never missed a therapy. And my goal with therapy is to get a 1/4 cup of mucus up everytime. And I don't stop therapy until I get that much up. If I can't do a social thing-I wouldn't. I'm not a monk! I have a social life, but my physio routine takes priority.
So thats what worked for me. I hope you find something!

No prob.
It was never explained to me that you had to force yourself to cough-It was my understanding that you just sat there on the vest, and if you coughed naturally that meant you had congestion, and great, spit it ip. If not, you were fine and I guess I thought the purpose was to just loosen mucus. When I switched hospitals they wanted to see me doing the vest. So I did what I normally do, put it on, sit there for 30 minutes, use two or three different frequencies and see if I cough at all.
Then they said there is a definate technique to the frequencies and to coughing-its not as easy as just saying well, if I cough great, if not, ok. I had to learn how to breathe in a way to force myself to cough, to do a controlled cough, and not put my airways into spasming. Like I said, the "textbook" way is-you should get up 5 spits for each frequency you do-you do 7-8 frequencies, thats 35-40 spits. Equaling about 1/4 cup of mucus per therapy. To get that mucus up, esp out of the deep airways is what keeps you living. The key to getting your pfts up, clearing your small airways and keeping lung damage to a minimum.

Heres an example of my therapy-
Start at frequency 7 for 4-5 minutes. Stop, deep breath in, (I blow out hard and fast just like a pft) Some people are worried about that giving them a spasm, so they go strong and slow when exhaling. When exhaling, put the pedal back on so your are vibrating as you exhale. Either way, whatever works best for you-it will bring up mucus. Do it 5 times in a row, giving you 5 good spits. (keep track of the spits in a cup)
Go to frequency 8/9-Do process again
Go to frequency 10/11-Do it all again-Repeat with frequencies 12, 15, 17, 20 as far as you can go. ( I only get to 18)

The whole point of physio is to get out as much mucus as you can, and get out mucus from those deep airway. Doing all the frequencies, from the little ones to the strong ones is what helps get out the deep mucus.
I hope this is clearer now, and let us know if you try it and if it helps at all!

Just wanted to add-after therapy this way, I feel like I just did a HARD workout-thats the way it should feel. Its difficult, thats probably why so many cf centers don't really teach it the correct way. I think they assme that people aren't really going to do this. But if you have enough incentive, you'll do it.
Hope you feel better!

WOW! I just learned a lot!!! I was doing the vest wrong.
I don't use it all the time because I exercise a lot! But wow this is great info. It is safe to do while pregnant right? I know I could ask my CF dr this but she has kind of dropped me since finding out I was pregnant sooo that is why I am asking you all. Thanks for the info!!!

Thank you guys so much for your responses.. I do need to exercise .. I have not been released to exercise yet.. I am clear to as of January 1st.... so maybe that will help..

As far as the Tobi. I haven't had any problems on it, but I have on Advair... so I stopped that.

Amy you are right.. The dream job, costs dearly.. Eventually, I want to go work part time.... and I am considering a move to a different state that would allow me to work part time and still own a home....

My health is paying the price for my passion, I am not even sure I care.. How is that for crazy? But I know you understand...

Christian thanks for the vest lesson.. I just recently readjusted my vest settings after Amy posted about what DR Warwick recommends..

Mel, Yup steroids are cut back.. I didn't even think of that.. I am still bitter about how much they messed up my lungs from being in the hospital..;

I actually have to post an update on that...

Thanks again guys, I appreciate it..

Jennifer

Everyone has to find that balance that works for them. I remember when my doctor said I had to get my pfts up hugely if I wanted to get pregant, I thought yeah, I'm gonna be doing 3 therapies (like the ones described above) per day, a sweatin cardio workout a day....and she was like no, no. It has to be a regimen that is comfortable and realistic to keep doing everyday. I hope you find that Jennifer. You've come so far-I understand why you are so concerned about you pfts, of course. But I also understand after all you've been through, why your job is so important to you.

Boxer 13-Are you a boxer or do you have a boxer? (i"m wondering b/c I have a boxer, she's 2 yrs old ) Yes, this regimen is perfectly safe for pregnant women. I'm doing it, and I consider it vital to a pregnant woman-it will keep my lung function up. My doc did say that around 6-7 months PT with the vest will likely have to stop. Then, I'll have to switch to having someone clap me, while I am upright or gently propped on my side, the doctor said. But Ill still do the coughing/spit technique. I hope this helps you! I was so happy to learn the right way to do the vest, esp when I saw such amazing results from it. Everyone responds differently, but of course it will do some good for sure. I'm concerned that your doctor dropped you-that is really not good. If you wouldn't mind-why dont' you come to the pregnancy section of this forum and write your 'story' with this situation. I'm sure everyone would find it interesting and informative, myself included-and maybe you can get some tips to help you out right now. wish you the best!!
