I use saline and a syringe too.
I don't tilt my head back. But i tilt it to the side. Like when one nostrill (sp?) is up and the other is down.
Then i push about 5 mills or a little more in the upper nostrill, let it sit there for a bit and tilt my head forward after that to let the saline go out.
This way i don't get anything in my throat. As the saline goes in the sinus cavity on the other (the lower) side. And when it's too much it leaks out through the lower nostrill..
This is what my ENT told me to do.
