I would also say: Don´t be a germaphobic, let him live a real life!
The only thing I would be careful with is the cleaning of the nebs as he will get every possible germ on it directly in his lungs. In my opinion the easiest but nevertheless safe way to sterilize the nebs is to rinse them and afterwards use a (hot steam)baby bottle sterilizer; the rinse washes away the remaining medicine, the steam (which has boiling temperature) kills germs. Dishwasher and hot water are not hot enough to kill germs.
