Tobra peak and trough's

I also get weekly drawings even when I'm only on it once every 24-hours. They sometimes make changes midway through for me, so then they do it more frequently once they change the dose (like a couple days after the change), but then back to every week.

How is Mark feeling? Is he home with you now? Take care of yourself too... I know you often put everyone else before yourself, so please make sure you rest and take care of you. <img src="i/expressions/face-icon-small-smile.gif" border="0">

Best wishes,

another thing to keep in mind is that Mark watches for his OWN changes while on tobra... for me, i started developing ringing in my ears about 3 weeks in and even though my levels were within normal range, they stopped it since tobra not only effects renal fx., but also the hearing. so, just make sure he is "in tune" to watch for such things. <img src="i/expressions/face-icon-small-smile.gif" border="0"> my docs were surprised all my levels came back normal yet my hearing was getting messed up, but they did not hesitate and put me on something else instead to finish my 5-week course of abx.

I only had mine done at the beginning of the course (4 weeks). Once established the rate of drug clearance shouldn't change that much. Doing it weekly is probably playing it safe for someone who has renal impairment to some degree, or someone who is doing dosing every 8 hours (tobramycin has a half life of about 2 hours with IV administration).</end quote></div>

I never heard of NOT having weekly testing. Anything could influence the toxicity that will cause renal issues....dehyration etc. Has anyone else NOT had weekly testing (granted I am referring more to home iv's then in hospital)?</end quote></div>

I did a little digging on pubmed and did find one NIH abstract on renal and auditory toxicity with high dose Tobra. The abstract can be found here : <a target=_blank class=ftalternatingbarlinklarge href="http://www.pubmedcentral.nih.gov/articlerender.fcgi?artid=174783
">http://www.pubmedcentral.nih.g...gi?artid=174783
</a>

I can have my doc pull the .pdf if anyone is interested in reading the whole study.

Also, I was on a relatively low dose every 24 hours, rather than 10mg/kg/day divided into equal doses every 8 hours. Perhaps they were a little less worried given my dosing schedule, my renal function, and my background. I've been relatively kind to my kidneys having only done IV abx twice. Those people who do abx several times a year would have to be monitored more closely. Perhaps peak/trough is not so one size fits all and some people can get away with less testing.

Thank you everyone for the information, this aspect of CF is so very new to both of us. Honestly I'm frightened, and worried, and I know Mark's nerves are wracked too over all of this.

Sonia, I will talk to Mark about watching for ringing in his ears. He's home now, he got out on Monday night and then I made him stay home yesterday. He went back to work today, said he was exhausted, but felt better on the lung aspect (hope that makes sense). My mom helped him with the kids tonight when I was at school, I don't think he's up to doing it by himself yet, especially since he can't really pick them up.

Thank you for all the support and answering all of my questions.
