I had a sputum cultrure stain and this is what it reads.

3+ Gram positive cocci

1+ Gram positive bacilli

Few PMNs

I had a doc tell me once that this says that I have bugs in my chest that are hard to treat?

The 3+ gram positive cocci is most likely a staph infection. The number 3+ means that when they viewed the sample under a microscope, they were able to see 11-25 of these little colonies growing.

1+ gram positive bacilli are probably C. diff. This can be a difficult infection to wipe out, which is probably the one your doctor meant when he said it's hard to treat. Have you been on antibiotics recently? Have you noticed a lot of mucus in your stools?

PMNs are polymorphonuclear lymphocytes. These are a type of blood cells that are the body's response to infection. The fact that there are few of them means you're probably in the early stages of an infection. Your doctor will probably start you on cephalosporin antibiotics.

Hmm..gram pos bacilli...C diff def fits into that category as well as Nocardia spp. They didn't have the culture results at this time huh? At least the doc can look at the type of bugs you have and give you what they call "empirical" treatment, that is they start you on antibiotics that focus on what your gram staining showed until they get the final results back with sensitivities to other antibiotics. The doc will then tweak your treatment accordingly. Gm pos. cocci especially if it is in clusters is staph species, like Lauren said. She did an excellent job describing what the numbers mean, quantity of what they micro saw under the scope.
Hopefully the final report will shed more light on what is growing in your lungs.
Hugs to you, Jenn <img src="i/expressions/face-icon-small-smile.gif" border="0">
